Summary

Fusion Maidstone is a health & wellbeing charity based in Park Wood. We opened our doors in 2003 and set about developing programmes, activities and services to address the changing needs of the residents of Park Wood with a strong focus on addressing health inequalities. Fusion subscribes strongly to the prevention agenda to improve and maintain health, wellbeing, avoid ill health, especially in an ageing and growing population. Fusion have worked closely with the community to develop a clear role as a trusted community hub. Our mission is and always has been to promote health, education, recreational opportunity, self-improvement and community support capacity of the residents of Park Wood and wider Maidstone area.

Purpose of the Job:
• To identify and directly engage with hard-to-reach individuals and families in Park Wood to raise awareness of Fusion.
• To provide responsive and effective support.
• To ensure immediate needs are assessed and met, respecting the wishes of individuals and families.
• Act as a ‘keyworker’ for the co-ordination and delivery of individuals and families support with the assistance of the wider Fusion team.
• Build a relationship with the KCC Community Wardens with a view to developing partnership working.
• Recruit and induct a total of 15 volunteers from the local community to assist with the project 5 in year 1, 5 in year 2, and 5 in year 3.
• Organise and deliver 4 engagement events each year of the project.
• Organise and deliver a minimum of 12 drop in surgeries each year of the project in various locations.
• Assisted by the Community Project Coordinator attend regular stakeholder steering group meetings.
Main duties and responsibilities:
1. To be professional, polite and well-presented whilst providing and maintaining excellent customer care at all times.
2. Work with people offering supported opportunities for them to improve their quality of life, develop their skills and make decisions about their future.
3. Provide general, practical and emotional support to people in order to empower them and promote independence.
4. Respond appropriately to challenging behaviour to promote the wellbeing of our service users, staff and volunteers.
5. Liaise with referral teams/partner agencies regarding referrals.
6. Participate in the staff team’s provision of individual support and needs assessment, including risk assessment, advice, emotional support and practical assistance.
7. Keep appropriate records of interventions with individuals and families and inform other staff of relevant issues.
8. Assist in referral pathway planning and making relevant appointments.
9. Lead on the formation of a project steering group consisting of stakeholders, residents, partners and Fusion staff, facilitating and hosting regular meetings.
10. Provide advice and assistance to people on personal budgeting, debt management and claims for welfare benefits. This can include a direct referral in to Fusion or our partner services.
11. Encourage peoples involvement in the development and operation of the service, facilitating appropriate consultation and participation.
12. Share responsibility for the effective use of information systems and procedures regarding people and other records.
13. Be a member of the staff team working to provide a safe, welcoming and supportive environment. Participate, with other staff, in providing a broad range of activities and services aimed at enabling people to live with reduced levels of support where appropriate.
14. Ensure compliance with appropriate Fusion safeguarding policies and procedures.
15. Actively promote equal opportunities and value diversity as both an employer and as a direct service provider, fostering a culture of fairness, equality and respect.
16. Understand Fusion’s 3 year business plan priorities and values of each partner and stakeholder. Contribute to the delivery of these in the provision of a high quality services.
17. To attend and participate in line management meetings and appraisals.
18. To undertake any other such duties and responsibilities as Fusion Maidstone, may from time to time, require.

This role requires an enhanced with barred DBS check.
