Summary


Blaenau-Gwent are seeking an exceptional health and well-being support worker to join our dynamic Integrated Community Resource Team. We are searching for individuals who are self-driven, passionate and eager to make a real difference to people's lives.

As part of our team, you will have the unique opportunity to help promote independence and empower individuals to achieve their personal goals. You will be working closely with the Community Resource Team and the Information, Advice and Assistance (IAA) team. You will have strong links to the hospital teams and promote prevention and early intervention.

The service operates seven days a week , 365 days a year between the hours of 07:30 and 20:00. The post holder will be expected to work a shift pattern that includes 12 hour shifts and weekend working.

Our CRT is a collaborative effort between the Aneurin Bevan Health Board and Blaenau Gwent Social Services, dedicated to providing individuals with the right intervention at the right time, from the right professional(s). As an Health and Wellbeing Support Worker, you will have the chance to make a real difference in the lives of adults in need of support to remain in their own homes. You will be an integral part of the wider multidisciplinary service comprising of Social Workers, Occupational therapists, Physiotherapists, Rapid Response Team, Falls Team, Community Ward's, Reablement and Home Care.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
