Summary

Senior Practitioner for Urgent Referrals Pathway. A new opportunity has arisen within the South Manchester HBTT to join the service as a senior practitioner with responsibility for accepting and managing the Primary Care urgent referral caseload. The post holder will be required to work 37.5 hours a week including including weekends, and bank holidays. This service operates over 24 hours, 7 days a week, and so there will be times when the post holder will need to work out of hours to facilitate line management and support practitioners. There is an expectation that the post holder will participate in out-of-hours bronze on call - this is subject to approval. You will be a senior team member providing clinical leadership, leading within our MDT meetings, and on clinical decision making within the service. You will support junior staff through supervision and service development. Key duties willinclude: Lead responsibility for overseeing development of the primary care urgent referrals pathway. Responsible for triagingreferrals from Primary Care partners Developing and maintainingrelationships and pathways with other services in Trust and with local partnership organisations. Provide clinical leadership, supervision, support and consultation to staff in the Home-Based Treatment Team in line with clinical pathway as outlined within the operational procedure. Providing in-house training and as directed to other services within the directorate Manage and evaluate a caseload in line with relevant clinical pathway as outlined within the operational procedure and will include: Act as an assessor undertaking bio-psychosocial assessments. Take responsibility for resource allocation and lead the development of the pathway with other community services. Line management and clinical supervision of staff members.
