To be professionally and legally responsible and accountable for all aspects of your own work, including the management of patients in your care. To accept responsibility for ensuring the effective selection, use and fitting ofequipment used both in the intermediate care beds and community (e.g. hoists). To communicate effectively with patients and carers to maximise rehabilitationpotential. Communication skills of persuasion, motivation, explanation and gaininginformed consent will be used with a variety of patients. Barriers to effectivecommunication will regularly be evident, e.g. sensory impairment, cognitive impairment, pain, emotional state, behavioural and communication difficulties. To be able to deal effectively and appropriately with patients/public who displayinappropriate behaviours (physical or verbal aggression) with the support and guidance from the supervisor. To ensure effective communication and collaboration takes place between yourself and members of the multi-disciplinary and multi-agency teams across hospital andcommunity. This may include verbal communication (e.g. case conferences, telephone) and written communication (e.g. report writing). To support others and recognise the limitations of your own clinical practice and seek the advice of others as appropriate. B5 Dietician To work within a multi-professional team in nutritional support in a community setting, providing support, care, and treatment to individuals, within guidelines and protocols identified for the role. To assist in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by following prescribed packages of care. To support other qualified staff in the management and execution of clinical practice. To carry out duties delegated and supervised by the accountable and responsible dietitians. To provide a dietetic outpatient services to patients under the care of their GP within the Cambridgeshire and Peterborough CCG. To provide a high standard of care. To manage own caseload of patients who require ongoing management and support with complex needs. Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting B5 Physiotherapist To work as an autonomous practitioner within the integrated multi-professional team, neighbourhood teams. To be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines, with the support of Specialist and Advanced Practitioners. To identify patient needs, agree goals and provide appropriate Therapy interventions. Provide adaptive equipment, rehabilitation programmes, and make recommendations for minor adaptations in line with current legislation. To keep the patient at the centre of care, ensuring patient and carer participation in decision making. Provide support and education to peers, new staff non-registered staff and students. To be accountable for a delegated caseload held by Integrated Care Workers and Assistant Practitioners. To provide a high standard of care. To work in various community settings, this includes lone working, with access to Specialist and Advanced Practitioners when required.
