Description

In this role, you will be responsible for conducting assessments and facilitating connections for individuals seeking assistance. As an Assessment and Connection Officer, your main responsibility will be to assess the needs of individuals and connect them with appropriate resources and services. Responsibilities
Conduct assessments of individuals to identify their specific needs and challenges
Develop individualized support plans based on assessment findings
Facilitate connections between individuals and relevant resources, such as community services and programs
Monitor the progress of individuals and make necessary adjustments to support plans
Provide ongoing support and guidance to individuals throughout their journey
Collaborate with internal teams and external partners to maximize resources and opportunities for individuals
Maintain accurate and up-to-date records of assessments, support plans, and progress reports
Requirements
Bachelor's degree in social work, psychology, or a related field
Minimum of 2 years of experience in a similar role
Knowledge of community resources and services
Strong assessment and problem-solving skills
Excellent communication and interpersonal skills
Ability to work effectively with individuals from diverse backgrounds
Proficiency in Microsoft Office Suite
Ability to manage multiple tasks and prioritize workload effectively
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £12.38
