Summary


We are recruiting a consultant geriatrician to join our exciting and rapidly expanding Frailty Team delivering integrated holistic care spanning both acute and community settings. The post holder’s primary duty will be to develop pathways and enhance the reactive frailty services, UCR, virtual wards, ED front door and Mary Seacole Unit. In addition, the post holder will have opportunities to support the integrated frailty service and be involved in providing reactive and proactive care across the acute and community areas. There will also be responsibilities in supervising and teaching trainees and medical students, as well as nursing and allied health professionals of the MDT. The post holder will drive forward developments, improving the quality of care delivered to patients with a focus on maximising peoples’ independence and supporting their return to their usual place of residence wherever possible. You will receive peer support from fellow Consultants working across the frailty services in Epsom General Hospital and Surrey Downs Health and Care and be fully supported by an integrated management team helping you to drive forward change.

Consultant geriatrician as part of the Frailty Team covering the following clinical areas:
• Community reactive services (UCR and VW) – providing clinical advice and leadership to the MDT. This may involve home visits of complex patients and liaising with other colleagues in the acute hospital, community teams and primary care. The post holder will provide presence at the team huddles and hold clinical responsibility for the case load (varies between 10-12 UCRs and 50-70 patients on VW). This is supported by a team that can be made up of GPs, specialty doctors, trainees, ACPs, PAs, nurses and therapists.
• Emergency Department (ED) – provision of clinical leadership within with Front Door Team. The post holder will be supported by the MDT, including junior doctors with the expectation that they will review approximately 4 new patients per day. There is also an expectation to in-reach to other medical wards to support early discharge of patients that could be managed on the virtual ward.
• Mary Seacole Unit - The post holder will provide leadership for junior colleagues as well as all other members of the multi-disciplinary team (MDT) present on the ward.
