Summary
To provide high quality nursing services including Chronic Disease Management (such as: Asthma, COPD, Diabetes), Learning Disabilities reviews, Long Term Condition reviews, NHS Health Checks, ECGs, blood pressures, bloods, foot checks, urine analysis, spirometry, ear syringing, cervical cytology, wound care and minor injuries management, general health and lifestyle advice, vaccinations, injections and immunisations. To be actively involved in health promotion to the Practice population in collaboration with the Primary Health Care Team, meeting the needs of the patients and supporting the delivery of the Practice Policies & Procedures. The post holder will work within their professional competence in accordance with the NMC Code of Professional Conduct and Scope of Professional Practice. To work under indirect supervision and perform tasks prescribed and delegated by the GP Partners, Practice Manager and Nurses working to PSDs, PGDs, agreed protocols, within an evidence base and within defined competencies to meet the needs of patients. To liaise with the Practice Pharmacy Team, Community Staff, Practice Nursing Team plus other healthcare professionals and to working closely with the Management, Administration and Care Navigation teams.
