Description
We are seeking a highly organized and detail-oriented Education Health Care Plan Assistant to join our team. As an Education Health Care Plan Assistant, you will play a crucial role in ensuring the successful delivery of health care services to students with special needs. This involves working closely with educators, parents, and health care providers to develop, implement, and monitor individualized education health care plans (EHCPs).
Responsibilities
Collaborate with educators, parents, and health care providers to develop EHCPs that meet the unique needs and goals of each student
Ensure that EHCPs are accurately documented and maintained, including regular updates and revisions as necessary
Coordinate and facilitate meetings between relevant parties to discuss and review EHCPs
Monitor and evaluate the effectiveness of EHCPs, and make recommendations for adjustments or improvements as needed
Provide training and support to educators and other staff members on the implementation of EHCPs, including strategies for supporting students with special needs
Act as a liaison between the school and external health care providers, ensuring that necessary medical services and accommodations are provided
Maintain accurate records and documentation related to EHCPs, including progress reports and evaluations
Requirements
Bachelor's degree in education, special education, or a related field
Strong knowledge of special education laws and regulations, including the Individuals with Disabilities Education Act (IDEA)
Excellent organizational skills and attention to detail
Effective communication and interpersonal skills, with the ability to build positive relationships with students, parents, and staff
Experience in developing and implementing individualized education plans or health care plans
Familiarity with various disabilities and their associated educational, medical, and social needs
Proficient computer skills, including experience with Microsoft Office and data management software
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
