What you'll be responsible for: Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties. Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing. Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings. Raising concerns to senior staff for support and management. Completing RiO documentation and planning care for patients. Presenting cases to Multi-Disciplinary Team for discussion around care pathway. Liaison with colleagues nursing, occupational therapy, medics, psychology and admin. The role will involve, but not be limited to you performing the above.
