Summary


Clinical Service Lead - Physical Health

This role sits within the Trust's corporate Physical Health team, which supports the delivery of high quality, evidence-based physical healthcare across the organisation.

This role holds operational responsibility for the Health Check Liaison Team, comprised of 7 x B4 Assistant Practitioners/Nursing Associates and 1 x B4 Team Administrator. The team work across community mental health services in Croydon, Lewisham, Lambeth and Southwark to support the delivery of physical healthcare to people with serious mental illness.

The role also involves supporting the Lead Nurse in the delivery of various clinical projects at a Trust-wide level to support quality of care. These may include policy, clinical governance, training/education, investigation and audit activities, depending on the needs of the service.

Applicants must have the ability to build positive and effective professional relationships, work with a range experienced leaders and be a credible nurse in practice. The role of Health Check Liaison Team Lead is an excellent opportunity for a nurse who is committed to the physical healthcare of patients. We will provide development, support and supervision.

The Clinical Service Lead will focus on the operational management of the Health Check Liaison Team, which includes building relationships with various stakeholders; bridging the gap between primary care and secondary mental health services and working innovatively to achieve outcomes.

Effective use of data is key to this role and the successful candidate will be proficient in data collation, analysis and report writing.

The Trust is dedicated to managing and developing a range of mental health services, working closely with service users and carers, our four partner local authorities, CCGs and NHS hospitals. Services are primarily focused on people with severe and enduring mental illness. However, it also promotes good mental health, early intervention to prevent more serious problems and provide support to primary care in dealing with people with less severe problems.

South London and Maudsley NHS Foundation Trust is a large and diverse organisation providing core mental health services to the Boroughs of Southwark, Lewisham, Lambeth and Croydon and specialist services both regionally and nationally.
