Summary

To assist in the implementation of a leading-edge bladder and bowel service for the assessment and treatment of children and young people. Provide continence interventions, toileting strategy, advice or treatment as relevant to the child's needs and capability taking into consideration any specific physical, mental or social needs of the parent/carer to engage with the service as delegated. Conduct continence assessments and follow an agreed care pathway to ensure effective treatment outcomes. Work in conjunction with other community staff to delivery individually structured management plans (self-management), and risk assessments. Evidence outcomes and the impact on the child. Respond to calls made to the Single Point of Access (SPA) so that parents/professionals can have access to advice and rapid referral to appropriate specialists within the team or be allocated appointments for assessments. This will involve offering specialist advice and guidance to the wider Children's Health 0-19 workforce and deputising for the continence lead nurse. This will be an 8am to 8pm service in line with the rest of Children's Health Service. To participate in providing a service within nurse led clinics, homes and schools as appropriate. Responsibility for overseeing effective work allocation, managing workflow and case holder responsibility. There is an expectation of duputising for the continence lead nurse in their absence. In conjunction with the Children's Continence Lead Nurse prevent unnecessary admission to hospital and reduce readmission rates for children with idiopathic constipation. To provide patients and carers with contact numbers if their plans need re-assessment before their next appointment is due. To ensure that all the re-assessments allocated to them are completed by 12 months and all necessary documentation completed to reflect this. Facilitate smooth transition from childrens to adults services to include young people aged 18, newly referred into the service. All children are to have transitioned by their 19th birthday. Raise any concerns or risks to the Children's Continence Lead Nurse. Liaise with GP surgeries as per pathway to provide prescriptions where an assessment has evidenced this need, ensuring that all assessments and information are shared with that practice. The GP will then complete the prescribing function. Share assessments and referrals to hospitals and acute services to aid care for complex health needs with an aim of returning to community/secondary care quickly. Be responsible and accountable for legible and contemporaneous nursing records using electronic records systems. Toileting skills assessments will be used to evidence where a toileting programme is the most effective care for the child or young person and should be seen as gold standard. Manage the care of patients requiring prescriptions of containment products avoiding overprescribing and selecting products that meet the child's assessed health needs whilst Children's Health Locality Manager Children's Continence Lead Nurse Children's Continence Deputy Nurse Children's Continence Deputy Nurse in line with the Lincolnshire Wide agreed formulary. To participate in clinical supervision as a supervisee as per policy. Provide clinical supervision for other registered nurses working within the wider Children's Health 0-19 service. To participate in the LCC appraisal programme as appraise. To deliver education programmes, seminars and in-service training days. Education is to be provided for all members of the Children's Health Service. Deliver training for local GP's, schools and community nursing/children's health colleagues. This includes nurses in special schools for children with physical disabilities and learning difficulties. Be responsible for identifying your own training and development needs. Requirement to work evenings and weekends as required and as part of a rota. To maintain professional registration within the Nursing and Midwifery Council Code, and complete revalidation. 15 To adhere to all infection control requirements as stated in LCC policy
