35-37 Monday to Friday, normal working hours
This opening assignment is for 3 months
City: DORCHESTER
Dynamic social worker with a strong value base and sense of person-centred practice. Have excellent communication skills, enjoy working in a fast-paced environment, have a flexible approach and be able to work closely with a range of professionals.
Be creative and passionate about enabling people to be as independent as possible. Need to be confident with the legal basis for practice and be able to demonstrate a good understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998 and Equality Act 2010.
Key Responsibilities
Undertaking strengths-based assessments, support planning and reviews to support customers to achieve their outcomes and promote their independence.
Working as part of a busy team and alongside other teams within the local authority.
Multi-disciplinary practice will be integral to the role.

Qualifications:
A degree in social work/Occupational Therapy or equivalent
Registered with the Health and Care Professions Council (HCPC)
Knowledge:
Working knowledge of relevant legislation & statutory guidance (Care Act, Mental Capacity Act, Children Act, Equality Act, human rights & data protection).
Experience:
Experienced in completing written assessments and plans and making evidence based and outcome focussed decisions using proactive risk management within set procedures.
Proven experience of delivering programmes and direct interventions which contribute improved outcomes for carers and the people they support.
Experience of appropriate sharing of information and sound understanding of GDPR/confidentiality.
Experience and understanding of adult safeguarding.
Experience of completing written assessments and plans.
Skills & Abilities:
Effective communication skills.
Ability to work as part of a team/multi agency partnership.
Able to reflect, analyse and deal with complex issues including risk management.
Ability to both support and challenge adults, families and carers as appropriate.
Inclusive person-centred approach.
