This opening assignment is for 3 months

Hybrid working may be available once fully familiar with the role, when demand allows

Enhanced DBS required for this role

A Senior Social Worker is required to work in the North Transfer of Care Team at the hospital in Huntingdon. Cover may be needed sometimes to assess people at Peterborough City Hospital and Queen Elizabeth Hospital Kings Lynn. There is the opportunity to work from home a couple of days a week once fully familiar with the role, however still may be required to come into the hospital on those days to support, if demand requires it. Able to hit the ground running so experience of Mosaic would be advantageous as well as good knowledge and use of Microsoft teams and excel spreadsheets.

Key responsibilities:

Triaging and allocating out all work that comes into the Hospital team which includes assessment of needs and safeguarding.
Providing sound legal and policy/case advice to workers. Leading and developing huddles.
Supervising social workers/adult support coordinators.
Proactively take the lead on team plan, safeguarding action planning and CQC assurance to support the team and team manager.
Deputising management tasks when team manager not in work.
Leading on complex cases and holding a small caseload as required.
Liaising with the Multidisciplinary team to ensure good team working with health.
Escalating cases appropriately to team manager/service manager/head of service.
Provide professional leadership in situations which have highly complex case arrangements for assessment and support planning in situations which involve risk, complexity and safety of the service user and others.
Assess and balance risk and protective factors within a safeguarding framework.

Qualifications:

Recognised Social Worker qualification.
Relevant specialist post qualifying and or post graduate training.
Current Social Work England registration.



Experience:

Experience of Mosaic desirable.
Experience of Quality assurance of work.
Experience of working as part of a multi-disciplinary team.
Experience of identifying and assessing need, formulating, and implementing effective social work interventions.
Experience of using information management systems to produce good quality data in a variety of formats.
Experience of negotiating to reach a resolution.
Experience of successfully effecting change.
Broad range of experience in multi-agency working.



Knowledge:

Proven knowledge, experience and understanding of needs of vulnerable adults including environmental factors and risk and protective factors.
Proven previous experience of summarising, analysing, and evaluating complex information
Knowledge of relevant safeguarding legislation, statutory guidance, and procedures.
Knowledge of the personalisation agenda – applying creative problem solving to maximise independence.
Knowledge and awareness of issues relating to communities from different racial and cultural backgrounds and Equal Opportunities.



Skills & Abilities:

Able to hit the ground running so experience of Mosaic would be advantageous as well as good knowledge and use of Microsoft teams and excel spreadsheets.
Able to make management decisions within budget constraints and know when to seek team managers authorisation as required.
Ability to write concise reports and have excellent verbal communication skills.
Ability to create a rapport and build relationships with clients and their families.
Ability to prioritise tasks, manage own workload and be accountable for case work.
Able to work with a person centred, strengths-based approach.
Ability to operate in a fast pace, changing environment.
Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
