Summary

CLCH Hertfordshire Adult Community Services -CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An opportunity has arisen for a Complex Case Care Coordinator to join our Complex Case Management Service in Hertsmere.

Hi, my name is Sandra and I am the Complex Case Care Coordinator. I have worked in the Hertsmere team for a long time and am really proud of the great care we give to some of the most complex and vulnerable people in our community.

The complex case management service takes time to understand people’s complex needs and develop personalised care and support plans, including scheduling multidisciplinary meetings, liaison with GPs and other community health services, and running a geriatrician led locality frailty clinic.

This role is approximately 50% patient facing, assessing patients and signposting them to the best services to meet their needs. The remaining 50% is office based at Potters bar Community Hospital, where you will be helping to schedule meetings, collect information about patients on our caseload, liaising with other professionals, and compiling reports.

We sit as part of the wider integrated locality team consisting of Community Nurses, District Nurses, Community Paramedics, Physiotherapists, Occupational Therapists, and Health Care Assistants.

The successful candidate will be enrolled into our 12 week induction programme, which will include working with the wider MDT, to enhance your skills and knowledge. You will have regular input from the Professional Development Nurses and myself to support you in gaining your competencies, skill and confidence.

We work Monday to Friday and can offer flexible shifts 8.30am – 4.30pm / 8am - 6pm. We will consider applications from either full time or part time candidates. There is a requirement to be available for work on Fridays to support our locality frailty clinic, and on Wednesdays to support our locality multidisciplinary team meeting.

Just as we care about our patients’ wellbeing, we care about yours!

We can offer you:
• A comprehensive induction into the community service followed by a local induction to introduce you to the role
• Car lease scheme *T&C’s apply
• Flexible working options
• Annual travel card loan
• Training, support and development in your career
