Summary


We are excited to be able to advertise a brand new post within Older Adult Community services. We are looking for a highly motivated and enthusiastic individual with community experience and a passion for working with Older Adults.

The post is a B6 Nurse Assessor and Duty Practitioner role for the Community Mental Health Team function of the Older Adult Integrated Community Team in Lancaster and Morecambe.

The successful candidate will take a lead in Assessing patients referred to this service and support with the Duty Practitioner function of the team.As a Registered Nurse you will be responsible for facilitating holistic, person-centred assessments that identify service users needs and risks whilst considering as a secondary mental health service how we deliver on meeting these needs.

We are an Integrated Older Adult team which includes a MAS, CHL and RITT function of the team but this post is for the CMHT function of the team.

We want to recruit people to the service who are passionate about working with Older Adults, share our values and are confident in their ability to contribute to the demonstrated effectiveness and high standard of care that the team provides.

It is a Nursing/Clinical role in a service that operates 9-5 Monday to Friday rather than 24/7 rotating shift pattern.

The main duties of the successful applicant will be to provide a comprehensive assessment, risk assessment and advice to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism.

The Nurse Assessor will understand the specialist interventions that the OA CMHT can deliver in accordance with care pathways and the Community Services Standing Operating procedure.

The applicant will need to be experienced in assessing the needs and risks of individuals, have a sound understanding of Physical health and mental health, be confident in recognising potential safeguarding issues and understand safeguarding processes and a working knowledge of mental health legislation such as the MHA and MCA and be able to maintain a high standard of clinical work.

The nurse will be part of the rota that provides a daily Duty role for the team.

Experience of working with those with Dementia would be desirable but is not essential.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
