Summary
An exciting opportunity to join Lancashire and South Cumbria NHS Foundation Trust to work in Pennine Physical Health Services as a Band 6 Specialist Occupational Therapist within the Community Rehabilitation Team. The team includes Occupational Therapists, Falls Specialist, Technical Instructor’s, Community Rehabilitation and Health Care Assistants. We work with the wider multidisciplinary team to support long term rehabilitation with patients in their own home’s to support increase or maintain their independence. We attend weekly Integrated Neighbourhood Team meetings to support and navigate referrals to the correct services with health, social care and voluntary sector partners.
We are looking to appoint a dynamic, enthusiastic Occupational Therapist to join our service. The role will involve assessing patients, planning treatment and coordinating support and equipment to meet the patient’s needs. The successful candidate will need excellent communication skills who is confident to work autonomously as well as part of a team.
You will be responsible for the assessment, treatment, planning and delivery of evidence –based rehabilitation interventions and provide clinical triage and support the management of patient demand through the system-discharging appropriately.
You will promote health and reduce inequalities for all patients accessing the service, so that they may be able to maximise their potential in terms of health education and social well-being.
To undertake comprehensive assessment of patients with complex presentations, using investigative and analytical skills.
To establish individualised management and treatment plans, using clinical reasoning and utilising a wide range of evidence based treatment skills and options to formulate highly specialised interventions and programmes of care, which may include working with staff across organisational boundaries.
Contribute to clinical leadership and supervision facilitating a culture of learning and reflective practice within Occupational Therapy.
To lead/contribute to training and development programmes as appropriate to the knowledge and skills of the individual, delivering to other professionals both within and outside the NHS and service users.
To participate in the LSCFT PDR review process ensuring business priority objectives are met and a personal development plan is maintained and evaluated. To cascade the PDR process to departmental staff as appropriate.
To be responsible for own continuing professional development and maintaining own professional registration and meet HPC/NMC Registration requirements maintain a professional portfolio and demonstrating evidence of reflective practice.
To have in depth knowledge of HPC standards and guidance, understand implications for and be able to apply to practice. E.g. HPC Standards of Conduct, Performance and Ethics HPC Standards of Proficiency.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Please note: Work is ongoing to transform adult physical health services across the Pennine Lancashire Footprint supported by commissioners within the Lancashire and South Cumbria system. This transformation aims to ensure service users in Blackburn with Darwen and East Lancashire receive equitable service provision from one service provider. From around 1stJuly 2024, the service provider will be East Lancashire Hospitals Trust. Staff employed by LSCFT will therefore transfer from LSCFT to East Lancashire Hospitals Trust around 1stJuly 2024 as part of this wider transformation plan.
