As a registered nurse assist in the provision of active rehabilitation, stepdown/up care within this 22 bedded inpatient environment.

The role with incorporate the assessing of needs, developing and implementing programmes of care and evaluating this input to support the facilitation of appropriate discharge back to community.

The post will support the Matron/Sister in ensuring effective and efficient day to day running on the centre ensuring safety at all times.

An exciting opportunity has arisen in our 22 bedded Rehabilitation / Frailty unit at the Cumberlege Intermediate Care Centre (CICC).

We are looking for a Band 5 Registered General Nurse to join our team . You will assist in leading our dedicated team in the provision of expert patient care and rehabilitation, so if you are looking for a challenge, this could be the post for you.

What is Staff Bank?

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

1. Clinical
• To use skills and knowledge to contribute to the assessment, implementation and evaluation of patient care.
• To use skills and knowledge to identify subtle changes in condition and to take clinical actions, reporting to senior nursing and medical staff any concerns.
• To ensure care is delivered in a safe and effective manner and as far as is practicable is based on an evidence of good practice.
• To delegate work to other non-registered members of staff in an efficient manner, prioritising work-load effectively and using appropriate and agreed tools.
• To be familiar with emergency procedures pertaining to community settings and how to instigate these as required.
• To contribute to the information required to support the Care Quality Commission inspection visits, and to be aware of the standards and be able to provide accurate information to inspectors
• To actively participate in the promotion of healthy lifestyles, early detection and prevention of disease.
1. Staff Management
• To facilitate compliance of other staff with regard to local and national standards of good practice.
• To communicate with all concerned to facilitate a smooth transfer of patients from hospital to the community as required.
• To assist in the delivery of learning opportunities that fulfils the development needs of colleagues.
• To contribute to the induction and training of students and other new staff.
• To work effectively as a member of the multi-disciplinary team to support the planning and achievement of local service objectives
• To contribute to the development of clinical governance and quality agenda for the service.
• To deputise in the absence of senior nursing staff and manage the centre as shifts require.
1. Communication and Working Relationships
• To participate in the Trust’s Supervision and Appraisal programme
• Attend team meetings as often as possible to ensure understanding and to assist in the promotion of a team where all members are respected and involved
• To use communication to ensure good standards of care are maintained and all members of the multidisciplinary team have the information they require
1. Professional Ethics
• To adhere to the Code of Professional Conduct as identified by the NMC.
• To facilitate compliance of other staff with regard to local and national standards of good practice.
• To exercise good practice with own time management, punctuality and attendance
1. Documentation
• To ensure up to date written and electronic records and activity data are maintained, in accordance with professional and organisational standards.
• To record accurately in Care Plans in line with Record Keeping Policy.
• To assist in the preparation of high quality written reports and any other written documentation as necessary to support patient care.
• To effectively use IM&T support systems as required i.e. Systmone and health roster
1. Leadership
• To assist in the implementation and evaluation of evidence based protocols, policies and integrated care pathways in line with the service provision.
• To assist in the regular review of patient information in conjunction with other team members, using a variety of mediums that take into account cultural diversity and communication difficulties.
• To demonstrate and pass on specialist skills to other staff both in the immediate team and wider colleagues as appropriate to their grade.
1. Education and Training
• To assist in the delivery of learning opportunities that fulfils the development needs of colleagues.
• To work within the multi-disciplinary team by assisting in the development, co-ordination, implementation and evaluation of teaching programmes for patients and their carers that provide them with necessary knowledge and skills to retain their independence.
• To contribute to the education and training programmes of staff within the team
• To contribute to the induction and training of students and staff
1. Service Development and Delivery
• To work effectively as a member of the multi-disciplinary team to support the planning and achievement of local service objectives;
• To support the implementation of changing practice in line with new targets and/or service developments.
• To contribute to the development of clinical governance and quality agenda for the service
• To actively engage in practice development, audit and research activities as necessary
1. Professional Development
• To demonstrate on-going personal development through participation in internal and external development opportunities, recording learning outcomes in a portfolio.
• To keep abreast of new trends and developments and incorporate them into practice as necessary.
• Demonstrate ability to review and reflect on own practice and performance through effective use of professional and operational supervision in line with local guidelines
