Summary


We are seeking to recruit a Band 5 Occupational Therapist to join our Later Life and Memory Service. We are a forward thinking, friendly and busy team aiming for high quality service provision. We are nationally accredited by MSNAP (Memory service accreditation).

We would like you to have some demonstrable experience of working within mental health setting and / or with adults with a dementia diagnosis and be keen to develop your professional skills in this clinical area.

As a community based practitioner you will work with some autonomy. Support and supervision will be provided by the team and Occupational Therapy colleagues as required. We already have two Band 6 Occupational Therapists within the team .

We would encourage an informal visit or telephone contact to further discuss the role.

Your main role will be completing OT functional assessments within the Memory Assessment role of our service and we are also re-establishing our CST groups which are being led by the Occupational therapists in the service.

As an integral part of the wide team there will be a requirement to complete some generic mental health assessments and interventions.

Responsibility for screening/assessments for OT based functional assessments/specialist assessment.

Responsibility for screening for Cognitive stimulation therapy, supporting with running the group work and the outcome measures utilised within this pathway.

To work as part of the wider MDT to ensure the highest standard of care is given to our patient cohort.

To complete mental health assessments/reviews as necessary.

To complete assessment of risks that may be presented to/posed by our patient cohort and plan for care accordingly.

To ensure that safeguarding is considered as part of the daily role.
