It’s an exciting time for our Urgent Community Response (UCR) Service as we continue to build on our successful model. UCR services work with patients in their homes across West Sussex communities to avoid hospital admissions and support those being discharged home.



Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver:


• A rapid response to people in the community who are experiencing acute episodes of ill-health.
• A supported discharge service to facilitate timely hospital discharges.
• Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.



We are looking for an Advanced Clinical Practitioner (Nurse or Allied Health Professional) to join us, so if you’re a dynamic, patient focused individual looking to take a lead role in a team that provides cutting-edge, excellent acute community care, we’d love to hear from you!

The focus of this role is to use your advanced clinical skills to assess patients, investigating and diagnosing their healthcare needs, ensuring they receive care and treatment that is compassionate and responsive to individual needs.

You will develop, utilise, and maintain skills beyond the scope of normal practice, such as advanced physical assessment, diagnostics, and prescribing. These will help you plan and direct the management of patients within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate to role.

You will lead on the strategic development of Trust-wide services in your specialist area, working with other strategic and clinical leads. Alongside this, you will lead and actively participate in education programmes designed to support other clinical staff in skills relating to the speciality.

An element of the role involves providing peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients demonstrating conditions relating to the speciality. You will also be responsible for insuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures. This involves being a competent research evidence consumer & to contribute to the establishment of protocols and guidelines for the development of the service. In line with this, you will also undertake and support research in the speciality.
