Summary

Clinical practice Provide Patient facing or remote Clinics for a long-term condition(s) Provide Patient facing or remote Clinical Medication Reviews for patients in their own home or care homes and provide advice to patients or carers in line with Medicines Optimisation principles (own vehicle required). Respond to medication queries from patients and staff and provide medicines information using appropriate sources referring to the Senior Clinical Pharmacist and/or GP as necessary in accordance with standard operating procedures. Provide medicines support to patients either face to face or via remote means. Be an active member of the Network Prescribing Group Contribute to the improvement of prescribing processes across the Network by providing advice to GPs, staff, and patients. Undertake medicines reconciliation in according to standard operating procedures (SOPs) and to update patients records accordingly. Perform medication switches in line with SOPs. This will involve reviewing patients records, amending records and informing patients if required. Support the achievement of the PCNs prescribing Quality and Outcomes Framework (QoF) targets. Undertake Quality Improvement initiatives and agreed medication audits, keeping records, and undertaking further cycles as required. Assist in the appropriate monitoring of prescribing budgets and to liaise with the Medicines Optimisation team at NHS Dorset Implementation of local and national guidelines and formulary recommendations Advise on medicines supply issues as appropriate. This will involve liaison with local Community Pharmacies and patients. Lead on and contribute to agreed service developments. Support the roll-out of electronic repeat dispensing. Use and maintain information systems and databases relevant to your position including incident reporting, medicines optimisation, prescribing data, cost savings and patient outcomes. Ensure that all relevant MHRA alerts are acted upon over an appropriate time frame and where appropriate to advise on any medication changes that occur as a result. Initiating contact with and responding to requests from patients, other team members and other healthcare professionals. To produce information to support any medicines awareness projects. Liaise with other pharmacy stakeholders such as NHS Dorset Medicines Optimisation Team, community pharmacies and hospital colleagues. Provide education and training to members of the multidisciplinary team and to support the pharmacy technician in their education and training.
