Summary

The role will be part of the current bed management team , working alongside a full time Transition of Care Nurse based at the Radbourne Unit. The post holder will support patients leaving the ward and being discharged to community into other care provisions or to their home address. Supporting patients to engage with local community based support networks to encourage and maintain independence within the local community when they no longer require an acute hospital bed. This will include working alongside social care and third sector providers to ensure the right place at the right time is achieved .
