Summary

Dacorum Healthcare Providers Limited (DHPL) is the provider arm of the Dacorum GP Federation. The Network Contract Directed Enhanced Service 2022/2023 and Enhanced Health in Care Homes Framework recommend the value of a Care Coordinator as part of the MDT to help support and deliver on the care home DES. Job Summary As PCN Care Coordinator you will work as a key part of the primary care network (PCN) multi-disciplinary team. You will ensure care is seamless and that everyone involved is working together. You will provide the capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with PCN primary care professionals. Operational · Proactively engage with care homes under the DHPL model. · Liaise with care home staff to ensure each resident has a personalised care plan. · Work closely with clinicians from the DHPL care home team to review patients. · Organise and co-ordinate referrals as appropriate, generate ICE forms as requested by clinicians. · Liaise with other teams within the wider system as appropriate to ensure holistic care is provided to all patients. Communication Be a good communicator, establish and maintain good working relationships internally and externally. Ensure practices, care home staff and clinical teams receive prompt communication regarding engagement with their patients. Promote a positive image of the service, both internally and externally. Participate in and contribute to service development initiatives, as appropriate or as directed. Ensure timely and accurate information reporting to management on agreed areas of work. Can communicate effectively to all relevant parties involved with the patient's care. Planning and Organising Supporting in the planning and monitoring of the care home service within DHPL Planning and organising team meetings when requested. Independent and effective management of own workload. Information and Data Co-ordination · To maintain accurate databases and spreadsheets in order to provide up to date information about individual care homes that fall under the service. Collect and cascade information for the care home service as requested. To understand and work with electronic data systems across General Practice, updating and disseminating information as requested. Care Home Care Co-ordination To promote integrated project working within the service. To refer to other agencies or individuals for those whose needs might appropriately be met elsewhere. To act as a resource person and assist other staff with information on available resources, relevant organisations to be approached. To provide and receive sensitive information about difficult or complex matters respecting confidentiality at all times. To manage and prioritise own workload without direct supervision. To organise administrative work; maintenance of files, electronic document management systems. Key Working Relationships The post holder is expected to develop and maintain effective working relationships with a range of stakeholders, both internal and external to DHPL practices. The stakeholders include, but are not limited to: Senior Services Manager Pharmacy Service Manager Senior care home pharmacist PCN pharmacists Lead care home GP GPs and Practice Staff Community, Health & Social Care Colleagues PCN Staff Clinical directors Freedom to Act Work within the boundaries of organisational policy Take action and own initiative Work autonomously on own areas of work seeking support from colleagues where expertise is required and escalating issues and concerns as necessary. To undertake statutory and mandatory training and other training as and when required.
