Summary

Please see attached Job Description for full details To co-ordinate the delivery and development of the Renal Transplant Service using effective evidence based practice in line with agreed national standards for patient safety and monitoring; To lead Specialist Nurse led clinics and be responsible for planning, co-ordination, delivery and evaluation of nurse led clinics; To be accountable for the planning and delivery of all aspects of patient care in relation to Renal Transplantation; To liaise with all key members of the multi-disciplinary team in the delivery of specialist patient care; To participate fully in the multi-disciplinary team meetings and work with the Consultant Nephrologist and Surgeons to deliver specialist clinical care appropriate to the needs of the patient; To continuously improve clinical care standards in line with the clinical governance framework; Demonstrate leadership in contributing to a culture of patient safety and continuous improvement in line with the clinical governance framework; Be accountable for provision of a specialist nursing service, using clinical skills in the assessment, diagnosis, prescribing and delivery of treatment plans to support all aspects of patient care across the pathway; Co-ordinate the provision of services to patients within specialist area, ensuring continuity of care across the Trust and between primary and secondary care; Play a key role in facilitating hospital discharge and preventing avoidable hospital admissions As an expert practitioner, be accessible to provide education and training and clinical advice and support to Trust staff, primary care staff, patients and relatives/carers; Participate in / lead on audit and research relevant to the needs of specialist area; Participate in service development within renal services across the Trust; Contribute to the achievement of national and local standards/targets; Support the Matron or Team Lead to facilitate co-ordination and delivery of a service which is both efficient and evidence-based.
