Summary

Job Purpose To provide specialist paediatric continence advice to children and young people, their families and carers and those who work with children with continence problems, through clinical practice, teaching, research and development, as part of an integrated team. To be responsible for the delivery of a paediatric continence service to children and young people referred from family/carers, GPs, health visitors, school nurses, other health professionals, social services and education. Dimensions To supervise continence link nurses, support staff and students working with the Post Holder. To supervise, educate and assess the performance of students as required ensuring the standard of practice and teaching meets the standards set by the degree level qualification. Key Responsibilities Clinical Practice Manage a clinical caseload of children and young people with complex continence problems in all clinical, social care and educational settings, maintaining accurate clinical records and data required by the service. Maintain continence clinics in a variety of settings. Assist with clinical management decisions required to ensure efficient, cost effective running of the Home Delivery Service. Liaise with Service managers on issues relating to continence promotion. Give clinical advice to nurses and other members of the multi-disciplinary team caring for children and young people with continence problems. Promote and demonstrate best practice, integrating evidence based practice, using an advanced level of clinical reasoning and clinical judgements. Within the scope of the service prescribe approved medication for children with continence problems. Ensure family centred care is practised, incorporating appropriate ethical and moral dimensions. Promote involvement of user groups and voluntary agencies in the development of services for children and young people with continence problems. Plan, develop, implement and evaluate educational programmes and training. Prepare and present written reports as required. Keep accurate records and statistics. Participate in the development, implementation and evaluation of research based nursing practice, policies and guidelines on continence care. Participate in the development and implementation of Audit of the Continence Advisory Service and continence care in all clinical, social and educational care settings. Initiate and/ or make a contribution to appropriate research into continence care Participate in professional meetings and forums. Professional Development Support and encourage reflective practice in continence care, promoting a culture of continuous improvement. Attend mandatory training. Participate in the PRD - appraisal process and take responsibility for reaching agreed development objectives. Maintain a portfolio to demonstrate continuing professional development. Participate in regular clinical supervision including safeguarding supervision in accordance with the organisations policy and procedure. Support the development, implementation and evaluate a continence link nurse/ worker schemes Contribute to the national professional debate within continence care through the appropriate Clinical Interest Groups (e.g. RCN Childrens continence and urology specialist nurse forum, ERIC) Maintain relevant resources related to current practice and research on continence care. Departmental Ensure that confidentiality is protected at all times. Promote and maintain best practice that ensures the health safety and security of self, child and young people and their families/carers and colleagues in accordance with the policies and procedures of the organisation. Demonstrate understanding of Clinical Governance and Risk Management and apply to the work situation Report and record all accidents or incidents as laid down in the policies and procedures of the organisation. Practice in accordance with procedures and guidelines for the Protection of Vulnerable Adults and Child protection. Management Work as a team member deputising for the Paediatric Continence Advisor and supporting other members of the Continence Advisory team when required. Participate in the recruitment and selection of clinical and non-clinical staff within the department. Assist with the induction, supervision and individual performance management of clinical and non-clinical staff within the department. NMC Statement All nurses are required to work within the guidelines laid down by the NMC in their code of professional conduct. Organisational To support the Head of Service in implementing targets and objectives to assist in the ongoing development of the Primary Care Team. To participate in service evaluation and advise the Head of Service on the service needs of the client population. To be responsible for management duties when requested by senior clinical staff. To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham City CCG. To support and manage the day-to-day delivery of the service within own service including prioritisation of support staff duties. Also organising own diary and time management (arranging assessments and appointments for own caseload). To ensure the Health & Safety of staff and clients at all times in accordance with Health & Safety legislation and Trust policy and procedures. To adhere to all other Trust Policies and Procedures. To attend team meetings and meetings with other agencies as required. To use information technology on a daily basis and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy. To act as a Duty Clinician and Duty Officer for the Lone Worker Tracking procedure (checking all staff have safely returned from visits and if necessary reporting missing clinicians to the Team Leader on call at the end of the working day). To undertake any other duties which may be reasonably required.
