Summary

Support Worker
County Durham - Community based.
Permanent, Full Tim (37.5 hrs)
Pay £12 per hour (£23,464 per annum) and great benefits including Health Cash Plan

As our Support Worker, you’ll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community and maintain their tenancy. Typical day as our Support Worker
• Assisting customers with practical tasks so they are able to continue develop their independence and maintain their tenancy, helping them to achieve the goals, aspirations and outcomes documented in their support plans.
• We support customers who are patients in our local Mental Health Hospitals in County Durham and Darlington and following discharge for up to 6 months in County Durham
• We support customers to find appropriate accommodation if needed whilst they are inpatients and in the community.
• Helping customers with daily living activities such as applying for benefits, budgeting, maintaining their tenancy, accessing social support groups and much more.
• Carrying out risk assessments, support planning, goal setting and regular reviews.
• We carry out regular support sessions with our customers and tailor these to individual needs.

• The ability to work on your own initiative, remain calm under pressure and have a resilient approach
• Recognise every customer is different and you’re able to adopt a flexible, personal approach, listening to customers to understand their needs
• To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

You’ll join Emma Caygill and our Mental Health Hospital Discharge team, we’re super passionate about what we do! We’re really proud of what we achieve as a team and how we support each other as well as our customers.
Job details
• Working hours are Monday to Friday 9-5.
• Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
• Although you’ll lone work some of the time, we do get together regularly for training and team meetings.
• Position is for a Community Worker based either at home in our Office at Belmont Durham or visiting customers in the community.
• Able to use technology to update support plans, complete online learning and to collaborate with colleagues.
• You’ll need an Enhanced DBS check done and we pay for that.
