Summary

Trust Junior Doctor in Geriatric Medicine

London North West Hospitals NHS Trust is seeking to appoint a Trust Grade Junior SHO level Doctor (FY2/CMT2) in Department of Medicine for Older People at Northwick Park Hospital to work on our Elderly care wards which focuses on a comprehensive geriatric assessment, and early discharge planning.

Our goal is to provide a continuum of care for Older People spanning the acute District General Hospital, the local Specialist Rehabilitation Hospital and the necessary medical, social and therapies provision to promote maximal independence of older people living in the community. We aim to treat acute illness appropriately, to enhance quality of life, to reduce disability, recognise when life is nearing its end, and to provide individualised person-centred care in the right setting at the right time, and in particular support:
• The management of older people in the acute setting and development of acute services specifically aimed at early intervention and admission’s avoidance
• Specialist, multidisciplinary Acute Wards for Older People at Northwick Park Hospital
• Links with STARRS and Harrow Rapid Response focusing on prevention of Hospital admission, ambulatory care, acute medicine, A&E and outpatient services for older people.

The post offers the applicant an exciting opportunity to gain experience in Care of the Elderly. The main duties of a junior doctor include:
• Comprehensive Geriatric Assessment of patients as part of a multidisciplinary team
• Providing the necessary communication back to GPs and family members
• Supervising the junior members of the medical team in day-to-day management of patients on the elderly-care wards
• Being involved in decision-making regarding care arrangements for older patients which may include case conferencing, best interest meetings and family meetings.
• Making presentations at meetings, and undertake audit and other clinical governance activities.
• Being actively involved in teaching and training within the department

We are commitment to regular appraisal and CPD. The successful candidate will receive training at SHO level, supervised by a qualified NHS consultant. The training plan will be tailored to suit the trainee’s educational objectives. They will be allocated a named educational supervisor and a named clinical supervisor for the tenure of their post who will meet regularly to agree and review short and longer term goals and personal development needs.

London North West University Healthcare NHS Trust (LNWH) cares for the people of Brent, Ealing, Harrow and beyond.

Our team of more than 8,200 clinical and support staff serve a diverse population of almost one million people.

We run major acute services at:
• Northwick Park Hospital: home to one of the busiest emergency departments (A&E) in the country. The hospital provides a full range of services including the country’s top-rated hyper-acute stroke unit and one of only three hyper-acute rehabilitation units in the UK
• St Mark’s Hospital: an internationally renowned specialist centre for bowel disease
• Ealing Hospital: a busy district general hospital providing a range of clinical services, as well as 24/7 emergency department and urgent care centre, and specialist care at Meadow House Hospice
• Central Middlesex Hospital: our planned care site, hosting a range of surgical and outpatient services and collocated with an urgent care centre.

We are a university teaching NHS trust, in recognition of the important role we play in training clinicians of the future and bringing the benefits of research to the public.

The department has acute frailty service which include a front door ED service and OPRACs (rapid access clinics). We also have a well-established orthogeriatric service and developing cardiology, surgical liaison and community services.

The Department of Medicine for Older People has a friendly and informal atmosphere where the junior doctors feel well supported and achieve their educational targets. The successful appointee will be allocated an educational supervisor to assure that a personal development plan is agreed and will assure progression through continued professional development. We aspire to high standards of medical and nursing care, and routinely assess aspects of this with regular clinical auditing, to which the new appointee will be expected to contribute regularly.
