An exciting opportunity has arisen for enthusiastic and motivated HCA to join the South East Essex Frailty Virtual Ward service. These important posts will work alongside a team of Consultant Geriatricians, Community Nurses, Therapists, and Health Care Assistants in the provision of the Frailty Virtual Ward working in the community, in the patient’s own home. You will be part of a high profile service to help alleviate current pressures in the NHS. The service will reduce demand for inpatient beds and on Emergency Departments and optimise patient experience by enabling them to receive safe clinical specialist care in the comfort of their own home.

What is Staff Bank?

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

FVW is an admission avoidance and early supported discharge team from hospital available 7 days a week that will help patients stay at home when they are feeling unwell rather than be admitted to hospital.

The FVW team provides specialist nurse and therapy led care in patients own homes. They visit patients after receiving a referral from a healthcare professional to plan ongoing care and treatment as appropriate.

The Band 3 Health Care Assistant will work as part of the FVW Team to deliver acute care in patients’ homes, including the monitoring of observations, venepuncture and supporting patients with personal care and rehabilitation during an episode of acute ill health.

KEY RESPONSIBILITIES CLINICAL
• It is accepted that at times, the HCA works without direct supervision and the Sister acknowledges accountability of all delegated care.
• Provision of high quality care to individuals, families, and groups in conjunction with other agencies, across South East Essex.
• To manage daily workload efficiently and effectively, taking into account the needs and preferences of the patient.
• To maintain practice to the standards set by the Care Quality Commission.
• Work towards promoting admission avoidance from primary care to the acute Trust.
• Provide care to a client group with acute and complex health care needs to ensure the highest standard of care is given which complies with current clinical guidelines and legislation.
• To observe record and report to the Sister, any changes in a patient’s condition or circumstances.
• To be familiar with the steps to be taken in medical emergencies and act accordingly.
• Work with the FVW Therapists to support the delivery of holistic care and admission avoidance interventions.
• Provide advice and support to home carers in the community on the FVW caseload



KEY RESPONSIBILITIES PATIENT / CLIENT CARE
• To undertake through each patient’s individual care plan, those duties delegated by the FVW Sister and therapists, which the Health Care Assistant has been trained and is competent and confident to perform. These tasks include: -
1. Personal care
2. Rehabilitation programmes
3. Monitoring of baseline physical observations
4. Blood glucose monitoring
5. Urinalysis
6. Phlebotomy
7. Cannulation
8. Male / Female catheterisation
9. Wound care management
10. Pressure area care
11. Administration of nebulisers and medication
12. Collection of specimens
13. Administration of insulin (on completion of competencies)
• Full training and competencies will be provided to ensure that the post holder has the appropriate skills to undertake these duties.
• To ensure that staff contribute to the information required to support the Care Quality Commission inspection visits, and that staff are aware of the standards and are able to provide accurate information to inspectors.
• Develop and maintain good working relationships with patients, carers, social care and other health care professionals to support multidisciplinary working.
• Maintain effective communication with the multidisciplinary team.
• Empower/enable patients and carers to maximise potential for self-care.
