Summary

Role Purpose To work as an autonomous health care professional providing care to patients requesting an urgent appointment on the day. You will assess, diagnose, treat, refer, and discharge patients presenting with a wide range of undiagnosed/undifferentiated conditions. The use of appropriate assessment tools to triage and co-ordinate care, including telephone triage, virtual E consultations and face to face consultations. Support, maintain and promote the efficient and continued development of same day services within the Practice. Role Responsibilities Clinical: Use high-level decision-making skills. Undertake consultation, ensuring patients receive high quality care, delivered in a timely manner. Undertake comprehensive history taking and physical examination and request, interpret results and investigations to reach a diagnosis for patients with undifferentiated/undiagnosed conditions. Provide assessment and appropriate management of minor injuries including chronic and acute wound management and suitable closure techniques, soft tissue and bony injuries, eye presentations and medical emergencies, referring to UCC and or involvement of other clinical practitioners as required. Undertake treatment nurse duties that have been agreed within the Practice. Prescribe medication that is appropriate to patient needs and in accordance with evidence-based practice, national, local and practice protocols and within the scope of qualified practice. Maintain accurate and contemporaneous records using the practice clinic system. To offer and support patients to adopt health promotion strategies that promote healthy lifestyles, and signpost to appropriate agencies when required and apply principles of self-care. Protect and promote the health and well-being of patients, their relatives, and carers. Work collaboratively with stakeholders across the Health and Social Care sectors, and in partnership with patients and/or carers, develop effective, efficient, and preventative care plans. Mentor and support other clinicians and participate in clinical peer reviews. Professional Role Maintain an up-to-date professional registration. Monitor the effectiveness of own clinical practice through the quality assurance strategies such as the use of audit and peer review. Maintain own clinical/professional expertise by participating in clinical activities, personal development planning, clinical governance, and revalidation. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained. Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Promote a professional approach at all times by dress and attitude to patients and colleagues. It will be necessary to attend and contribute to nurse meetings as requested. Communication To be aware of data protection issues. To liaise effectively with other health professionals both inside and outside the organisation, with regard to accepting and making clinical referrals. Communicate effectively with patients and relatives, considering differences in language, literacy, and culture.
