Summary
Utilise advanced clinical and diagnostic skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs of both Adults and Children in order to develop a comprehensive short term care plan aiming to optimising health and wellbeing of patients and where possible to prevent avoidable attendances/admissions to secondary care. To act as role model ensuring that assessment and treatments conform to best practice as agreed through organizational or nationally accepted NHS guidelines issued from approved sources such as NICE. To undertake the first contact autonomous role through face-to-face client assessment and consultation. Request or perform and interpret tests using clinical judgment and expertise.
