Summary
The post holder is responsible for the assessment of care needs, planning, implementation and evaluation of programmes of care for patients Out of Hours, exercising a higher level of clinical judgment, discretion and decision making. The post holder is responsible for the leadership of a team of community nurses. You will play a key role in demonstrating innovative practice, service improvement and maintaining the continued professional development of team members and learners in the clinical environment. If successful this vacancy will require an enhanced DBS check at a cost of £41.90.
