Your Job Family Purpose Roles in this family require people to be qualified (fully or working towards) healthcare professionals who are responsible for designing and delivering a range of health and care services and advice to patients (face to face, by telephone or by developing written materials for patient and customer portals / apps) Your Role Purpose To work in conjunction with assigned designated hospitals/centres to deliver education to patients families and care givers in the safe administration of parenteral nutrition, biologic therapies and intravenous medications and to administer these medications to children as required. To deliver services in a customer focused, patient centric manner that optimises therapeutic adherence to treatment and provides a positive patient experience, whilst complying with relevant quality requirements. Benefit, maximises Our Values Your Key Deliverables Conduct training interactions to ensure patients family or care givers are able to safely administer medication prior to patient leaving hospital. Undertake a clinical assessment of patients to ensure they are well enough to receive treatment, including completing any required observations such as o2 saturation, blood pressure and heart rate. Provide on-going support to patients through the combination of support programmes to include telephone, clinic-based and home visits as per product and client protocols. Report adverse drug events as per protocol e.g. informing pharmacy and client nominated clinical contact. Our Regulatory Responsibilities Adhere to (CQC/ GDP/NMC/GPHC / ICO) standards relevant to role Chair / Attend all relevant committees aligned to remit of the role Be aware of all responsibilities relating to Infection Prevention and Control Your Skills and Knowledge Knowledge and Application Undertake a clinical assessment of patients to ensure they are well enough to receive treatment, including completing any required observations such as o2 saturation, blood pressure and heart rate and ensure any deterioration in the patient is managed appropriately. Set up parenteral nutrition infusion pumps and ensure medication is safe to use. Flush off central lines after infusion is complete. Recalibrate pump settings for new prescriptions based on weight and blood results. If patient is unwell, assess whether a hospital stay is required and arrange transport to hospital as required. Remain with the patient on journey to hospital to ensure full aseptic procedure is followed to limit infection. Liaise with families to determine level of nursing support required and alter scheduled visits in line with needs and contractual agreements. Report any non-compliance with therapy to client contact, usually Hospital Clinical Nurse Specialist. Complete clinical evaluation form (CEF) following each patient contact. Provide on-going support to patients through the combination of support programmes to include telephone, clinic-based and home visits as per product and client protocols Take bloods to be sent to labs for analysis. Conduct roles and responsibilities in accordance with NMC Code of Practice and Sciensus policy, procedures and protocols. Report adverse drug events as per protocol e.g. informing pharmacy and client nominated clinical contact. 20. Maintain accurate clinical notes, and keep care plans up to date Skills/Abilities: RSCN or RN-C Experience in a Paediatric Nurse setting, either in hospital or community environment Clinical skills required include clinical assessment, administering sub-cutaneous injections and cannulation. Also phlebotomy skills would be desirable. Strong understanding of therapies relevant to service offering, including NHS knowledge relating to the management of patients with treated conditions. Pays close attention to detail and follows rules and protocols; quality oriented. Strong communication skills, including actively listening, communicating sensitively, putting people at ease, and making technical details easy to understand.
