Summary

To work collaboratively within the Post Covid Multi-disciplinary team to identify appropriate patients for referral into social prescription. Play an active part within the MDT by offering expertise on personalised support to individuals to take control of their wellbeing, live independently and improve their health outcomes. Ensure suitability of referrals and follow up with one-to-one contact with the patient to discuss the referral, making home visits where appropriate within their organisations policies and procedures. To develop trusting relationships with patients by giving people time and focusing on what matters to them. Adopt a holistic approach to delivery, based on the persons priorities, and the wider determinants of health. Co-produce a personalised care and support plan to improve health and wellbeing. To use health improvement information to help people with Post Covid Syndrome to self-manage their health and encourage behavioural change. To support individuals to recognise and change their current lifestyle and to identify how their way of life may affect their health and wellbeing. To help individuals navigate services and activities that are available to them in their community and make informed choices. Where appropriate, physically introduce people to appropriate community groups, activities, and statutory services, ensuring they are comfortable, feel valued and respected. Follow up to ensure they are happy, able to engage, included and receiving good support. Develop strong links with a wide range of local VCSE organisations, community, and neighbourhood level groups, utilising their networks and building on whats already available to create a menu of diverse community groups and assets, who promote diversity and inclusion. Provide sound knowledge on work related issues, which include training, work schemes and rights at work. To support individual wellbeing plans over an agreed time scale and review and revise wellbeing plans with individuals as appropriate. Build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity, and lifestyle choices. To be aware of sensitive barriers to adopting a healthier lifestyle such as affordability, accessibility, and life circumstances. To use motivational interviewing techniques to encourage people to engage with opportunities for physical and social activity and to encourage and motivate people, using creative ways of addressing negative behaviours. To maintain a high level of confidentiality in all aspects of work including patient records and conversations and maintain comprehensive records of patients services. Where required and as appropriate, refer patients back to other health professionals for additional support. To build networks with local community resources such as activities and services that may have an impact on health and wellbeing. To develop multi agency working across communities and voluntary and statutory organisation to improve health and address health inequalities. To identify means of development of the service to improve the health and wellbeing of the local community and identify barriers that individuals who are experience Post Covid Syndrome are facing. Play an active part of the local peer support network for Social Prescribers. Undertake health/other professional and patient engagement activity to promote the service, in line with their employers own requests.
