Your job as an Advanced Clinical Practitioner or Primary Care Paramedic covers a wide scope, and ultimately allows autonomy in the role, with clinical support from our Practice Team when it is required. Main duties of the job Your clinical practice is the main focus of your advanced practitioner role at Hilary Cottage. Your duties include: Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe (if you are a prescriber) and review medication, appropriate to patient needs, and within scope of practice Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Overview of your organisation We are in Fairford - a small Rural town in Gloucestershire, close to the Oxfordshire and Wiltshire borders. We are a friendly supportive team of around 35 people. Hilary Cottage Surgery has been a part of the Fairford community for many years - currently run by 3 GP partners. We are a small rural practice with around 7500 patients on our books. We have 7 amazing GPs, a bunch of amazing receptionists, a brilliant dispensing team supported by our super pharmacists. Our admin team are fabulous, and there aren't enough superlatives to describe our incredible nurse team. If you're not in Fairford, you would enjoy a short rural drive to and from the surgery - and if you are local you can walk to work. With a focus on everyone's well-being at work, we are supportive and friendly from top to bottom, and (we think) a great place to work! In addition to the Clinical Practice responsibilities already described, you will: demonstrate courageous and critical thinking and be a caring, compassionate and committed experienced practitioner (nurse or paramedic) who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures be an excellent communicator, demonstrating your verbal, written and listening skills with patients and your colleagues maintain accurate patient records at all times utilise appropriate available technology so support efficiency and effectiveness in your role
