Nurse Case Manager

Currently we are expanding as a business which leaves us looking to recruit a passionate experienced Nurse Case Manager to join our team and propel Routes along their journey into the future of Healthcare.

Snapshot:

Role: Nurse Case Manager

Hours: Full time hours 35+ per week with additional on-call hours required on a rota basis.

Location: Across LANCASHIRE. Generally home based with some required travel.

Salary: negotiable depending on experience.

Due to the nature of our business and the role the successful candidate will require to be in possession of a full valid UK driving licence with access to their own vehicle.

Role Overview:

This new exciting, fresh opportunity has arisen to become a key part in the Routes family and be hands on in ensuring high quality standards.

As Nurse Case Manager you will be seen as a wealth of knowledge and support for your team and be expected to:

To have continuing responsibility for setting and monitoring standards and quality of care delivered to service users.

To monitor and ensure high standard of record keeping in accordance with the NMC, national legislation and local standards enabling effective communication with other multi-professionals.

To develop and maintain a culture of person-centred care within the community and throughout Routes Healthcare.

To take ownership and manage a caseload of complex clients and manage their care package both clinically and holistically.

To develop, review and implement service user care plans and risk assessments in line with CQC regulations and best practice guidance, and to assist in developing the skills of non-qualified staff to support you.

Overseeing and managing staff within the care package.

To ensure the ongoing competency of field staff, carrying out workplace observations, supervisions and one-to-one support sessions as required.

To co-ordinate and lead the transition, start up, and continuity of all complex care packages; involving client, MDT; managing the clinical training needs of the care team, and maintaining appropriate skill mix to ensure safe and effective clinical practice.

Overseeing and managing clinical interventions and staff training.

Experience:

- Registered Nurse Band 6 or above. (Essential)

- Full UK Driving Licence and own vehicle. (Essential)

- Willing to commute across the designated area. (Essential)

- Tracheostomy and vent. Experience. (Desirable)
