Are you an existing or an aspiring Advanced Clinical Practitioner?

Are you passionate about improving services for frail and vulnerable older people?

We are seeking highly motivated and experienced nurses or allied health professionals to join our existing team of ACPs, covering pathway 2A and 2B rehabilitation beds in Ashbourne, Belper, Heanor, Ilkeston and Ripley.

A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice (or equivalent) and who can demonstrate contemporary experience working at an advanced level within a relevant clinical environment.

A Band 7 trainee position may be offered to an aspiring Advanced Clinical Practitioner who is committed to undertaking an apprenticeship training programme.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people within a community care setting.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of the older person.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

If offered a trainee position you will commit to undertaking an apprenticeship programme comprising a 3-year MSc in Advanced Practice and the development of an ePortfolio to demonstrate your capability across the four pillars.

You will be uplifted to Band 8a on successful completion of the MSc, successful submission of their portfolio to a cross-organisational review panel.

You will be required to travel between different sites to deliver clinical care on a regular basis and therefore access to transport is essential.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
