Summary


This post is fixed term until 31stMarch 2025 due to - meet the needs of the service

Working with the Value Based Health Care Team, there is an exciting opportunity for an Independent Prescriber pharmacist to join the diabetes optimisation team to increase the prescribing of SGLT2i’s in patients with type 2 diabetes.



The role involves:


• Undertaking diabetes specific mediation reviews in GP practices, to optimise therapy in accordance with project objectives and protocols.
• Providing education and information to primary care teams.
• Contributing to the project protocols and liaising with primary care


This is a fixed term post until 31stMarch 2025, starting immediately, 12 hours per week, reporting to Elizabeth Jenkins.
• Must have an Independent Prescribing qualification.
• Must have a knowledge of prescribing medicines for the treatment of type 2 diabetes.
• Previous knowledge of primary care clinical systems.
• Previous experience in running patient facing clinics.
• Be able to collect and record any patient interventions made.
• Liaise with the Lead Clinical Pharmacist for the project, pharmacy technician and other members of the team.
