An exciting opportunity has arisen within the Division of Intermediate Tier Services, rated OUTSTANDING by the CQC, we are seeking an advanced clinical Practitioners to join the Extensive Care service, predominately to work within our Acute frailty service this is a blend of Community and Acute hospital work, within our new acute frailty unit.

Extensive Care is an Umbrella term as the service has a number of sub services within it.

These are, an intermediate bed base within the Stamford Unit, this is a 96 bedded Unit over 3 floors. This is a blended approach to care with Therapy GP and an ACP workforce. We are also very proud to have are new Urgent Crisis response team that work across our Neighbourhoods and support our Award-winning Digital Health Service, Virtual wards, along with having very strong ties to our intermediate urgent care team, this is a truly integrated team consisting of Nursing, Therapy and Adult social care. The Extensivist service is well established over 5 years, and sees patient in the community who have multiple hospital attendances, along with long term conditions and complex health needs. We are really excited to be developing our acute frailty service and Community frailty service


The service main focus is to reduce unnecessary hospital attendances, treating patient at home where clinically safe to do so, whilst providing exceptional standards of care

We have a strong team of ACPs within the service that are supported By a Team of band 7 Enhanced Nurses & Paramedics.

We are fortunate to have a team of Band 4 Assistant practitioners that are very experienced and support the team greatly.

We have Trainee ACPs that are at various stages of there education

We are supported greatly by our GP Consultant who works within our service and is leading on our Community frailty service, he works closely with Professor Martin Vernon, who is our frailty consultant and lead for the service.

Our is Hugely supported by our two Nurse Consultants that provide training and development within the team.

Whilst developing and supporting new initiatives across the service & the wider Intermediate Tier services.

Main duties of the job
• Registered Nurse / paramedic / Therapist with a current registration
• working at an Advanced Practitioner level
• Independent non-medical prescriber
• MSc in Advanced Practice or Equivalent
• Car and driving licence essential
• NMC Approved mentor preparation course or equivalent

The successful candidate will provide highly specialised care within the community and Care setting, to those patients with chronic and acute conditions using advanced clinical knowledge that is evidence based

The Advanced Practitioner will function as part of an integrated Care organisation and will demonstrate competence in advanced assessment, management and treatment.
• Provide expert advice and to ensure the needs of the patient are met by leading, challenging and changing practice within the community.
• Work collaboratively with other health care professionals in primary and secondary care to ensure a high quality easily accessible service.
• Provide a community wide lead in the development of the role of the advanced practitioner and the service provided
• To accept new and follow-up referrals
• To pro-actively support and maintain patients within the community and care home setting
• To facilitate discharge of patients back to their place of residence as appropriate
• To undertake clinical audit and research
