Summary

We are currently recruiting for a Care Coordinator position for our client based in Cheshire. The Care Coordinator ensures clients receive the highest quality of care through completing person centered care plans, identifying risks and carrying out regular staff observation and competency assessments. The key responsibilities of our Care Coordinator include:
To provide leadership to care staff
To provide care in accordance with current best practices, according to Policy and Procedures, agreed standards, legislative requirements, relevant regulations, under the direction of the Manager
To supervise the care delivery within the service
To maintain skills at a current level, and undertake such training and development as may from time to time be required to maintain practices as up to date
Ensure that Service Users are at the heart of the care delivery and their wishes and preferences enhance their well-being
Plan and allocate care assignments to appropriate care workers, with due regard for the particular needs of the individual and the skills and attributes of the worker.
To organise and coordinate care delivery in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager and within the financial plans.

Ensure that the care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards.
Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centered packages of care.

Dealing with queries from clients and families in a calm and professional manner.
Carry out on-call coordinating and response duties outside normal office hours as agreed.
Be responsible for promoting and safeguarding the welfare of those individuals they support

Other duties associated with the role.
Take part in the On Call out of hours’ service as required.
To provide cover for colleagues during absences.

Who we’re looking for
Eagerness to learn and the ability to implement feedback through training
Be able to follow instructions and work in a team
Someone who is kind, patient and understands the principles of good care.
Strong organisational and time management skills.
A social care qualification such as Level 2 or 3 Diploma in Health and Social Care.
Previous experience.
Good attention to detail.
DBS required, however immediate requirements will need to be covered by someone with a transferable or very recent certificate.
