Social Worker - Level 1 - Adults
Description
We are seeking a compassionate and dedicated Social Worker - Level 1 to join our team and provide support to adults in need. As a Social Worker, you will play a crucial role in empowering individuals to overcome challenges and improve their overall well-being. You will be responsible for assessing clients' needs, developing personalized care plans, and connecting them with necessary resources and services. Additionally, you will collaborate with other professionals, such as healthcare providers and community organizations, to ensure a comprehensive and holistic approach to client care. The ideal candidate will possess excellent communication and interpersonal skills, as well as a deep understanding of social work principles and practices.
Responsibilities
Conduct initial assessments to determine clients' needs and identify appropriate interventions
Develop and implement personalized care plans in collaboration with clients and their support networks
Provide counseling and emotional support to individuals and families experiencing difficulties
Advocate for clients and connect them with community resources, such as housing assistance, healthcare services, and job training programs
Monitor and evaluate clients' progress, adjusting care plans as needed
Collaborate with interdisciplinary teams to coordinate client care and ensure a holistic approach
Maintain accurate and up-to-date client records and documentation
Requirements
Bachelor's degree in social work or a related field
Valid licensure to practice as a Social Worker in [state/province/country]
Proven experience working with adults in a social work setting
Strong knowledge of social work principles, practices, and ethics
Excellent communication and interpersonal skills
Ability to establish rapport and build trust with diverse populations
Proficiency in using computer software and electronic health record systems
Ability to maintain confidentiality and adhere to ethical guidelines in all client interactions
