Social Worker - Standard Adults (Grade 7)
Description
We are seeking a highly motivated and compassionate Social Worker to join our team as a Standard Adults Social Worker (Grade 7). In this role, you will have the opportunity to make a positive impact on the lives of adults in need, providing them with the support and resources necessary to improve their overall well-being.
As a Standard Adults Social Worker, you will be responsible for conducting assessments, developing care plans, and providing direct counseling services to individuals and families. You will work closely with other healthcare professionals, community organizations, and government agencies to coordinate and facilitate access to services and resources. Additionally, you will advocate for clients' rights and assist them in navigating complex systems.
Responsibilities
Conduct comprehensive assessments to identify clients' needs and determine appropriate interventions
Develop and implement individualized care plans in collaboration with clients, their families, and other healthcare professionals
Provide counseling and support services to individuals and families, addressing a wide range of issues such as mental health, substance abuse, domestic violence, and homelessness
Collaborate with community organizations and government agencies to coordinate and facilitate access to resources and services
Advocate for clients' rights and assist them in navigating the healthcare and social service systems
Monitor clients' progress and make appropriate adjustments to care plans as needed
Maintain accurate and up-to-date records of client interactions, assessments, and interventions
Requirements
Bachelor's degree in Social Work or related field
Must be a licensed Social Worker in the state
Minimum of 2 years of experience working with adults in a social work setting
Knowledge of local, state, and federal regulations related to social work and healthcare
Strong communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Proficient in using electronic record-keeping systems
Hours Per Week: 35.00
Start Time: 09:00
End Time: 16:30
Pay Per Hour: £35.00
