Summary


Occupational Therapist - Band 5 - Merton Enhanced Therapy Service,

Are you a Band 5 OT or a highly motivated individual who completed OT qualification recently and looking for an exciting, new challenge and a chance to take on OT responsibilities in the community ? We are looking for a dynamic, motivated individual to join our Band 5 AHP staff supporting a large, MDT community therapy service.

Enhanced Therapy Service facilitates rapid hospital discharge by providing rehabilitation to Merton Clients. The team comprises of AHP therapists, Assistant Practitioner’s and support workers. It provides urgent pathways to adults that are being discharge from hospital, working with them to ensure their safety and regain / maximise their independence.

You will be a occupational therapist or a person who completed OT recently with excellent clinical skills who is a team player with a positive attitude and commitment to providing excellent holistic community based care.




• To clinically assess the referrals from the hospitals for patients who are ready to be discharged, but who have ongoing health or social care needs.
• To provide local clinical support and advice for the Band 3/4 and the wider MDT in the CSU and be responsible for the education and supervision of rehabilitation support workers and MDT students.
• To be responsible for line management and supervision of junior staff within the service
• To be responsible for own caseload including highly complex cases and aware to request support from Senior members when required.

* To support in developing, implementing and evaluating a high quality Enhanced Therapy Service, ensuring that patients receive the highest standard and timely care within the trust.



Possession of a UK driving license and access to a vehicle for business purposes is essential. Successful post holders must be able to work 37.5 hours across 7 days per week and at several locations where the services require it. The service operates between 8am and 6pm.

To manage caseload, prioritise referrals, assess, plan and implement intervention autonomously or with the guidance of senior therapists, ensuring clients and carers receive optimum level of therapy within available resources.

To undertake OT/holistic assessments using evidence-based outcome measures, in a variety of settings i.e. patients home or care home setting.

At all times to work clinically within an evidence based framework

To be responsible for gaining valid, informed consent for assessment and treatment interventions, and sharing of information following legal and professional guidelines

Following specialist assessment, to use good clinical reasoning and judgement to identify functional concerns which require investigating, analysing and assessing.
