Dementia Advisor Manager Vacancy
Dementia Concern supports people living with dementia and their families or carers in Ealing. We provide specialist information, holistic advice and practical services in the community, and are commissioned by West London Health Trust.
We have an exciting vacancy to join our team and deliver high quality frontline services. The role is office based with home visits and travelling around the Borough of Ealing.

Dementia Advisor Manager - Full Time - 35 hours per week.

Applicants must have previous experience with vulnerable adults or older people, and be able to imagine how dementia effects a person and those they love.
Salary: Dementia Manager £30,000
Holiday Allocation: 25 days per year
Line managed by: Director of Services

JOB DESCRIPTION

JOB TITLE: Dementia Advisor Manager
RESPONSIBLE TO: Director of Services
SALARY: £30,000
HOURS: 35 hours per week

THIS POST REQUIRES AN ENHANCED DBS.
Main aims of the post
The main purpose of the post is to manage and develop the Dementia Advisory Service for people with dementia and their carers along with your own case load. As part of the management team, the post holder will also work with other managers and the Services Director on organisational and service developments and represent the charity as appropriate.
Manager Tasks

The Dementia Advisor Manager will:
• Form links with external agencies e.g. the Carers’ Centre and other stakeholders
• Carry out file reviews for Dementia Advisors as appropriate
• Undertake training to develop team leading skills
• Check the Single Point of Access (SPA) dedicated e-mail address for referrals.
• Attend Operational Meetings as might be required
• Perform regular supervisions with your team to support and guide them.




Main Tasks
The main tasks of the post are to:
1. Carry out home visits to build trust and thereby assess and identify the social, mental and physical needs of people with dementia and their families or carers.
2. Liaise with health and social care professionals to advocate for the clients’ needs – thereby promoting independence and involving clients in all decision-making.
3. Inform and assist people in obtaining a formal diagnosis of dementia.
4. Refer back, as necessary, to cognitive impairment services e.g. for medication issues or changing symptoms relating to dementia, and inform the GP of referral.
5. Refer to partner agencies for welfare benefits applications as appropriate.
6. Provide ongoing emotional well-being support for people with dementia (and their families or carers) by phone or home visits, including for those living alone.
7. Conduct ongoing monitoring of the social, mental and physical health of people with dementia and update GPs and other agencies following each review.
8. Educate people with dementia and their families or carers, so that they can have choice and control over their current care plans and future care needs.
9. Undertake related duties occasionally, as requested by the Director of Services.
10. Keep detailed records of all actions undertaken.
11. Attend appropriate support, supervision and training.
