Summary


We are seeking a dynamic and suitably qualified and experienced health care professional to support the strategic delivery of the Greater Manchester Child and Adolescent Lead Provider Collaborative (GM CAMHS LPC). Working as part of a wider multi-professional programme team across all providers, to ensure quality responsibilities are met, and to support the collaborative and to be self-assuring and self-regulating.

As a Case Manager in Greater Manchester CAMHS LPC the post holder will work as park of a dynamic team in delivering an effective service that oversees all care pathways for young people who require inpatient Mental Health Inpatient care within NHS and Independent sector providers.



The post holder will provide case management direction for a caseload of young people and will over see the assessment, intervention and discharge planning throughout their inpatient journey. The role will include robust oversight of the access assessment, whilst ensuring young people are placed in the least restrictive environment with goal orientated treatment.

The role is also pivotal in the Quality Assurance processes that are required for all 5 providers within the partnership. This will involve leading on quality and environmental assessments & reviews, visiting providers to monitor service provision is of high quality, liaising with service users, carers and advocates, and collating relevant reports and dashboards in order to provide assurance to the Trust Board and NHS E/I
• Provision of clinical expertise to the LPC
• Ensuring effective care pathways, within and across LPC boundaries
• Resolution of complex or disrupted care pathways in conjunction with colleagues across England (and within UK for patients placed in services outside England)
• Support the LPC in ensuring that services are compliant to all specifications and guidance.
• Ensuring the delivery of high quality services that are value for money

• To provide clinical assurance of the quality of the care provided by specialised services within the Greater Manchester provider collaboratives;
• Supporting the team in the assurance of specialised services compliance to applicable specification and guidance;
• The post-holder will provide the link to the Greater Manchester Integrated Care System (GM ICS) and locally commissioned services to provide a co-ordinated approach to care and discharge planning, which results in robust and effective care packages;
• Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved;
