More detail about the role As a Registered Nurse working in the Community you will provide person centred care, based on the holistic Nursing assessment in order to plan, implement and evaluate Nursing care. You will promote and support self-management and work with people to manage their long-term conditions, preventing Hospital admission, and support early discharge from the in-patient setting. The role also focuses on using and developing your Clinical skills and supporting individuals at the end of life. You will work in a Community Nursing Team which is part of the wider Adult Health and Social Care Team, as well as working with a wide variety of providers, to meet the Nursing needs of individuals in their home setting or alternative residential settings. Experience in Community Nursing is desirable but not essential, as full training will be provided and fully supported. Within Community Nursing, career development opportunities include education in specialist areas, such as Tissue Viability, End of life care and Long-term Conditions, with link Nurse roles supported to enable Continual Professional Development. We also provide opportunities to study for the Specialist Nurse Practitioner qualification, which is currently being awarded over a two-year programme to successful applicants.
