Summary

This list of duties and responsibilities, which follows, represents a broad range of responsibilities, which may be required to be undertaken either routinely or periodically, with consultation and agreement by the post holder Key working relationships Clients/Patients PETRUS Hostel staff RBC Adult Care GMP Non-statutory agencies Voluntary Agencies Mental Health Services Podiatry Drug & Alcohol Services Dental Services GP Practices Single Point of Access Service Treatment Room Service NWAS & Urgent Care Centre RHA Management Team Responsibilities underpinning the role To develop a good working relationship with key partners listed above To attend meetings as required To provide advice and guidance on health issues to the staff at the Hostels To ensure quality services and take account of service risks, with robust clinical assessment and care planning skills Clinically skilled in assessing and managing wound care and arranging referral to the Treatment Room Service Provide written updates/reports/presentations on the progress of the service as required To deliver the required clinical skills and competence to fulfill the role of the HART Outreach Nurse To develop a good working relationship with the wider health and social care teams to enable streamlined, timely care and response for the homeless and transient of the borough To plan and organise the post holders own workload, including contribution to audits/project work, and training sessions for Hostel staff as appropriate Ensure records are managed in line with GDPR & RHA policy To record personally generated information through robust systems of working. (Personally generated information includes information and records relating to audit and clinical work undertaken by the post holder] To maintain registration as a registered nurse and comply with appropriate professional codes As appropriate to the post, maintain and develop professional competence and expertise, keep up to date with clinical evidence and opinion, and local and national service, legislation, and policy developments, agree objectives and a personal development plan and participate in the appraisal process with your line manager To undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager All employees should understand that it is their personal responsibility to comply with all organisational and statutory requirements (e.g. health and safety, equal treatment and diversity, confidentiality and clinical governance)
