Job Title: Occupational Therapist/Physiotherapist Grade: Band 6 Ward/Dept: Therapy Location: Gosport War Memorial Hospital, PO12 3PW Trust: Southern Health NHS Foundation Trust Working Hours: 37.5 hpw in a 7-day service Pay Rate: £21.80 The Role An exciting opportunity has arisen for an experienced Occupational Therapist or Physiotherapist to join our dynamic inpatient rehabilitation teams at Gosport War Memorial Hospital. The post holder will be providing high quality care and rehabilitation to patients with a range of functional and rehabilitative needs. The Trust/Ward/Department Southern Health provides a wide range of mental health, physical health and learning disability services for people across Hampshire. Our aim is to work alongside the people we support and our health and care partners, to constantly improve and deliver the best possible care. Gosport War Memorial Hospital provides a range of inpatient and outpatient services for the local community. Our Community Physical Health Services provide physical health services across much of Hampshire both in the community and in our inpatient services. We have a number of care pathways, based on different conditions (such as physiotherapy or respiratory) which we use to provide very tailored treatment for our service users. The Responsibilities If you are successful in your application to this post, you will be expected to manage and deliver on the responsibilities outlined below as well as those not included here, call today to discuss further: Undertake assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of treatment programmes. Provide expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care To provide clinical Occupational Therapy/Physiotherapy skills within the Community Therapy Team/ Community Hospital Team(s), and all other provider organisations. Assess and treat a range of complex factors relating to the patients condition, diagnosis and prognosis To provide specialist intervention tailored to meet the uniqueness of the individual. To be successful in applying for this role you will need: Enhanced DBS Degree level qualification or equivalent To be registered with the Health and Care Professions Council (HCPC) Experience of working as part of a multidisciplinary team NHS experience Rehabilitation experience Previous experience of working in a community setting Person Specification Qualifications Essential Relevant degree or equivalent HCPC Registered Essential Criteria Essential Enhanced DBS In addition to experience, all Qualified roles will be required to provide evidence of Relevant Qualifications Registration with the relevant Registration Body if applicable.
