Summary
We are looking for a Support Worker to join our team and provide a person centred, needs led service to adults with a learning and/or physical disability.
We are looking for caring, enthusiastic day staff to work at The Chestnuts, Surrey.
You will be required to assist with all activities of daily living and support the residents to participate in a range of activities within the home and community.
MAIN PURPOSE OF THE ROLE OF SUPPORT WORKER
o To effectively work with your colleagues to promote the health, independence, choice respect, dignity, rights, safety and welfare of the residents at all times. To act in a professional manner as a representative of the Company at all times.
o To support the Home Manager, Deputy Manager and Senior Support Workers in the smooth running of the home.
o To work as part of a team with all your colleagues.
o To provide personal care to all clients, promoting independence and respecting their dignity at all times.
o To complete all daily, weekly and monthly records and documentation as required.
o To comply with the residents care plans, risk assessments and guidelines at all times.
ABOUT THE SERVICE
The Chestnuts provides residential support and accommodation for up to twenty adults with profound and multiple learning disabilities (PMLD), physical disabilities, autism, communication and sensory impairments and complex needs.
Each individual is supported to develop a person-centred plan and health action plan that promotes independence, giving people increased choice and control over their lives.
