Home First supports the approach of “why not home, why not today?” Wherever possible patients should be supported home for ongoing assessments where this is not possible alternative pathways are available. Staying in hospital for longer than necessary has a negative impact on patient outcomes and ensuring patients are given the chance to continue their lives at home is vital for their long-term wellbeing .

The Recovery @home service forms an integral and essential part of Discharge to Assess, being available to provide timely care and support to facilitate discharge from hospital and onward assessment and identification of need. The team will work closely with health and social care services.

The service provides a 7 day / week service from 8am to 8pm working across both the community and hospital settings. Staff are required to work flexibly over the full week.

The Senior Rehab Support Worker role will function as part of the new and exciting Recovery At Home team by overseeing and supporting care and rehabilitation delivery within a community setting. The Recovery at Home service aims to provide a seamless service and to support discharges from hospital or prevent admission to hospital. The post holder will undertake a range of delegated tasks plan and will report to the Team Lead. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care.

It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams with focus on clients’ need.

You will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, and working with colleagues to evaluate interventions. Working as part of a multidisciplinary team, community teams and Social Prescribers, you will be a key part of our patient support team.

The Senior Rehab Support Worker role will support other professionals in coordinating all key activity including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centered.

• Complete skilled screening assessment for clients referred to the service, prioritising in consultation with the wider Multi Disciplinary Team (MDT)
• Carry a delegated caseload of clients with a range of needs, within a defined clinical / technical area of expertise, and be responsible for their assessment and treatment. To analyse the results from this and enable the treatment plans to be adapted in consultation with the wider MDT
• Undertake skilled therapeutic technical rehabilitation using innovation, reporting client progress back to the wider MDT and making minor alterations to client treatment progress, receiving guidance from the registered staff when required including Physio, OT, nurses and medical team.
• Work independently in the implementation and evaluation of occupational therapy and physiotherapy interventions for clients within the specified clinical / technical area of competency and in a variety of settings.
• Provide effective feedback to the wider MDT, including changes in the patients’ condition and any relevant information appropriate to the patient care, utilising judgment and analytical skills as to the appropriate action to be taken within their delegated area of responsibility.
• Assist the MDT in evaluation of client intervention and rehabilitation progress using appropriate outcome measures and evaluation tools.
• Utilise skills to identify need for, fit and review specialist equipment and adaptations to promote independence, rehabilitation and mobility with patients,
• Provide planned therapeutic activities and to work alongside other staff utilising personal skills to provide excellent client care and deliver treatment programmes on a one to one basis or in groups.
• Complete risk assessment and continually evaluate risk throughout rehabilitation in relation to all aspects of safety regarding self, client and colleagues.
