We are looking for an enthusiastic and driven professional to join our dynamic team who have been involved in and have a track record in delivering projects which improve patient care and help our clinicians to do their jobs. Recent projects include;
• Establishing the Frailty Hospital @ Home service
• Virtual Hospitals
• Delivery of a 12-month pilot service focused on advance care planning in care homes
• The roll out of digital solutions such as Isla Care.
• Integrated Respiratory Service transformation
As a senior member of the team, you will lead the Transformation Team and ensure all transformation priorities are delivered on time.
You will work closely with stakeholders from across primary, secondary, community, and social care, as well as wider stakeholders and the community to deliver transformation initiatives.
