Summary


An exciting opportunity has arisen for qualified children and young peoples psychological wellbeing practitioner to join #Thrive, Rochdale’s Award Winning Emotional Health and Wellbeing Service.

The service works with young people in Heywood, Middleton, Rochdale and the Pennines to provide emotional health and wellbeing up the age of 19. The service is unique in its existence in Pennine Care offering an integrated support offer with Tameside and Oldham and Glossop Mind and Your Trust.

The service is in high demand locally with young people not requiring to go through the usual red tape of a referral to access support. In true collaboration with our young people they discuss their needs and agree through joint decision making their goals and treatment plans.

Working alongside colleagues in locality and single point of access as well as camhs young people can transfer seamlessly as their needs warrant to prevent the need to tell their story more than once.

As a CYPWP you will work alongside mental health practitioners and counsellors as well as a dedicated small admin team. You will be fully supported in your role by the Team Leader and the the extended CAMHS services.

This opportunity is a fantastic opportunity for an already established CYPWP or someone wishing to take the next challenge in their career.

• Accept referrals via agreed protocols within the service and develop programmes of care packages.
• Undertakes patient-centred assessments, which identifies areas where the young person wishes to see change and or recovery and makes an accurate client assessment of risk to self and others.
• Support decisions with the wider team on suitability of new referrals, adhering to the department’s referral protocols, refers unsuitable clients on to the relevant service or back to the referral agent as necessary or steps-up the person’s treatment to high intensity psychological therapy.
• Provide a range of information and intervention for evidence based high-volume low-intensity psychological treatments. This may include guided self-help, behaviour management, computerised CBT, information about pharmacological treatments. This work may be face to face, by telephone or within a group setting.
• Educate and involve parents/carers family members and others in treatment as necessary with the clients consent.
